Note: Depending on your device and PDF application, it may not be possible to fill out the PDF digitally on a smartphone. We therefore recommend completing the form on a computer.

Please send us the completed and signed form together with your samples. Each sample must be packaged separately, clearly labelled and sealed in an airtight container (e.g. a resealable plastic bag).

You can drop off your samples in person or send them by post.
